WAVE on Wheels Program Request

Thank you for your interested in a WAVE on Wheels Education Program.

This form must be completed for EACH program date being requested. Consecutive programs can only be scheduled on the same date at the same location with the same animal.

Please allow 2 weeks after submission for WAVE to determine animal ambassador availability and send a quote for your chosen program(s). Due to risk of avian flu, penguins are not available for programs until at least April 20, 2026.

Your school or organization may be eligible for scholarship funding to cover some or all of the cost for a WAVE on Wheels program.

At least 75% of our children receive Free and Reduced Lunch assistance.At least 75% of our children receive Free and Reduced lunch, AND we have funding to cover a portion of the program cost.My school or organization does not meet any of the criteria above.
To receive scholarship funding, you must explain how a WAVE on Wheels program will impact your participants and enhance learning. Include demographics on underserved populations that will participate in the program. If booking for a non-school community organization, please include the name of the closest public school and the free/reduced lunch percentage rate.

If your organization is awarded a scholarship/grant funding, you agree to complete a post program survey to be eligible for future funding.

Submission of this request form does not constitute a scheduled program. You will receive an email confirming your program(s) date and time within 1-2 weeks.

If you want programs on multiple days or at multiple locations, please complete another request form specific to date/location.
WAVE on Wheels Program Cancellation Policy: Cancellations received 15 or more days before a scheduled program will receive a full refund minus a $25 administrative fee. Cancellations received 14 days or less days prior to the program date will incur a $100 cancellation fee. Exceptions may be made on a case-by-case scenario. If a program can be rescheduled, a rescheduling fee may apply. Rescheduling should be done through your original contact. All cancellations must be made via email to wave@wavefoundation.org. Any refunds will be credited to the original form of payment. Program, cancellation, and rescheduling invoices will be sent via WAVE QuickBooks/Intuit. If an organization does not pay fees incurred by cancelling or rescheduling, WAVE reserves the right to refuse future program scheduling requests. If WAVE must cancel a program due to the organization not abiding by program guidelines, the booking organization will be liable to pay a fee up to 100% of the program cost.*

WAVE on Wheels Program Guidelines: Please keep in mind that we allow a maximum of 30 participants in the PreK program and 50 participants in Classroom Program sessions. Please keep in mind that we allow a maximum of 125 participants, including staff, in our Assembly Program sessions. This helps us to provide the best experience for your participants, the animal ambassador(s), and WAVE Education Staff. Program participant limits must be observed or may result in program forfeiture and/or additional program costs. Meet & Greet sessions allow guests to come to our cart/table and ask questions about our animal ambassadors (not a formal seated educational presentation). There is no limit to the number of participants for a Meet & Greet program. Please assist to ensure all guests get a chance to visit the Ambassador Animal during program times. Your organization must have a representative(s) providing adequate supervision of attendees for the entire program session. We require level paved ground and ramp/elevator access into the building and presentation space for 30"W x 60"L mobile aquatic carts. The cart requires electricity. Please provide access to an electrical outlet within 15 feet of the cart in the presentation space. Our Education Team will arrive approximately 15-30 minutes prior to the start of your program. Please allow this time for uninterrupted set up to ensure the program begins on time. Our programs involve the transportation of materials and live animals. To reduce the set-up time and stress on the animals. One consistent presentation space is required for all program sessions. For the safety and welfare of Penguin animal ambassadors, only WAVE staff are permitted to touch Penguins. Penguins will require 15 minutes to acclimate and can be viewed for a maximum of 45 minutes. If more than one session is scheduled, a 30-minute rest period is required. Penguin and Education staff must have access to a quiet location away from others for the break. Please provide a 6-foot or 8-foot table. We require that the indoor and/or outdoor environment maintain a temperature between 70 - 85 degrees Fahrenheit. Outdoor presentations do require shade (stable tent or permanent structure) and must be dry. Outdoor venues do require an indoor back-up plan, or the presentation may be canceled. Animal ambassadors must be kept away from other animals (including service/support), excessive noise (speakers), and air pollution (smoke). Animal ambassadors are living creatures. We try our best to bring the requested animals. However, due to the needs of our animals, we cannot guarantee a specific species for a program. WAVE Foundation reserves the right to cancel a program at any time. As an accredited member of the Association of Zoos and Aquariums (AZA), Newport Aquarium supports animal choice and control. For example, the Penguin may choose not to enter the transport container at the Aquarium. It may want to stay in a transport container instead of standing in public after arriving at your location. If we arrive at your facility and the penguin chooses not to participate, our WAVE staff will still deliver an educational program. Thank you for understanding and helping protect this endangered species.*

Filling out this document, hereby releases and forever discharges WAVE Foundation and Newport Aquarium, and all of their officers, directors, employees, contractors, volunteers, parents, subsidiaries, and agents (in both their individual and representative capacities) (collectively, the “Releases”) from and against, and hereby waive, any and all claims, liabilities, damages or expense, including reasonable attorneys’ fees, that may be alleged or incurred in any way related to or arising out of any person’s participation in any program or related activities provided by WAVE Foundation and/or Newport Aquarium, including, but not limited to, any and all claims, liabilities, damages or expense arising out of the negligent acts or omissions of any or all of the Releases. In addition, you give print, social media, photo, and video release consent to those participating in the program. WAVE Foundation reserves the right to cancel this program at any time, in which case, a full refund will be granted. We will then contact you to confirm your visit and discuss any details. Animal selection cannot be guaranteed and may change.*
